However, it is important to recognize that these activities are often performed with different goals. Accident investigation tends to focus on the “what” and “where” of an event while failure analysis attempts to answer “why” and “when.” After an investigator develops a clear understanding of the event, he or she will often identify a root-cause. Root-cause analysis is the process of understanding “how” a sequence of events was first put into motion.

For many projects, the failure of a single component or “link” can lead to a chain reaction of subsequent failures. Other times, failure could result from seemingly unrelated factors. Failed components often leave behind evidence that can be interpreted by trained professionals. Much like a detective, an investigator must be able locate crucial evidence that identifies the root-cause and suggests improvements that can prevent similar accidents in the future. Addressing a problem at the root is critical to engineering design and product improvement because fixing a problem at its source usually results in the greatest improvement to the overall design. This approach is analogous to a Doctor treating the cause of an illness rather than the symptoms.
